Stephenville Medical & Surgical Clinic, PA, Community Health Clinic, and River North Pharmacy comply with applicable Federal civil rights laws and do not discriminate on the basis of race, color, national origin, age, disability, or sex. Stephenville Medical & Surgical Clinic, PA, Community Health Clinic, and River North Pharmacy do not exclude people or treat them differently because of race, color, national origin, age, disability, or sex.
 
Stephenville Medical & Surgical Clinic, PA, Community Health Clinic, and River North Pharmacy provide free language services to people whose primary language is not English.
 
If you believe that Stephenville Medical & Surgical Clinic, PA, Community Health Clinic, and River North Pharmacy has failed to provide language services, or discriminated on the basis of race, color, national origin, or age, you can file a grievance with Vickie Gill, Civil Rights Coordinator, at 254-968-6051. We are located at 150 River North Blvd. Grievances must be submitted in writing, within 60 days. Complaints must be submitted in writing, and include 1) name 2) complete address 3) phone number (including area code) 4) email address (if available) 5) name of the person you believe discriminated against you 6) brief description of what happened, how, why, and when you believe rights were violated 7) additional relevant information 8) signature of the person filing the complaint 9) date of complaint. If you need help filing a grievance, Vickie Gill is available to help.
 
Once a grievance is received, our Civil Rights Coordinator will conduct an investigation, taking steps to keep information about the grievance confidential, and issue a written decision within 30 days of receipt. The decision will include information about your right to appeal. Appeals must be filed within 15 days of receipt of written response, and we will respond to the appeal within 30 days of receipt of the written appeal.
 
Stephenville Medical & Surgical Clinic, PA, Community Health Clinic, and River North Pharmacy will not retaliate against anyone filing a complaint, participating in a grievance investigation, or who opposes discrimination.
