Thank you for trusting me with your child’s health care needs over the last 21 years. It is with mixed emotions I announce I am leaving Stephenville Medical & Surgical Clinic (SMSC) and Community Health Clinic (CHC) effective March 31, 2023. There are several providers at SMSC and CHC you may seamlessly transition care to, including: Haley Banks, MD, Josh Carswell, PA, Cortney Fanning, FNP, Benjamin Marcum, MD, or Clayton Roberts, DO. Your medical records will remain here at SMSC/CHC. If you would like to have your medical records transferred to a provider at a different location, please call our office at (254) 968-6051 ext. 2226 to make arrangements for authorization to transfer your records.
